Pennsylvania’s Performance-Based Contracting initiative represents a dramatic shift to how intellectual disability and autism providers operate and deliver services. LumiCare uses a comprehensive assessment process to determine the technology, support, and training needs of each person and works with individuals and their families/support teams to create customized support plans to ensure long-term success.
LumiLink Nurse Connect Services
LumiCare offers fully customizable nursing solutions designed to deliver timely, comprehensive, and effective care. Through LumiLink Nurse Connect, providers have 24/7 remote access to RNs, trained specifically to serve individuals with an intellectual disability or autism through a HIPAA-compliant telehealth portal. LumiCare works with providers to more effectively distribute nursing resources, conduct critical health monitoring activities, and intervene early to address individual health conditions. LumiCare is able to ensure that all LumiLink services are appropriately documented within a client-selected EHR system.
Pharmacy Solutions
Life Tree Pharmacy customizes medication management with personalized cycle, packaging, and delivery options to accommodate the unique needs of diverse populations in settings such as hospitals and transitional facilities, nursing homes, youth facilities, assisted living residences, and home and community-based residential sites. Life Tree’s comprehensive technology and administrative infrastructure allows providers to access consultative services from a licensed pharmacist, and provides the ability to access patient information and place orders. Life Tree’s primary pharmacy management software has interfacing capabilities with an extensive list of commonly used eMARs and eHRs.
LumiLink/Life Tree Integrated Care Model
LumiCare and Life Tree Pharmacy provide a comprehensive solution for clients requiring nursing, pharmacy, consulting, and project management services. LumiCare offers tailored technology solutions, from telehealth integration to ongoing remote support, while Life Tree ensures personalized medication management across diverse care settings. Together, they can deliver a fully integrated care model that streamlines nursing care, medication management, and project execution, enhancing health outcomes and fostering client independence.

Merakey DDTT offers trauma informed training and consultation services to providers that are supporting or looking to support individuals that are dually-diagnosed with a mental health disorder and intellectual and developmental disability. These services include intensive workshop training, on-going breakout sessions with individuals who complete the training, and individualized consultations to address the specific needs of an agency or individual.
Clinical Consultation Services

Voithos offer Trauma Informed Care (TIC) training and consultation services to providers that are supporting or looking to support individuals that are dually-diagnosed. These services include intensive workshop training, on-going breakout sessions with individuals who complete the training, and individualized consultations to address the specific needs of an agency or individual.
Clinical Consultation Services
Voithos offers Clinical Consultation Services which include a comprehensive case review process led by experts in the areas of IDD, autism, behavioral health disorders, trauma, and behavioral support services. These services are designed to enhance the lives of individuals, reduce the need for psychiatric hospitalization or the use of restrictive procedures, and help families and/or interdisciplinary team members further understand the complexities of the individuals with whom they serve. A specialized and targeted training curriculum is available to enhance staff competencies to effectively serve individuals with complex needs.
